Gan Mai Da Zao Tang (Licorice, Wheat and Jujube Decoction) gān mài dà zǎo tāng · 甘麦大枣汤

Safety. In terms of ingredient count this is among the smallest formulas in the classical repertoire, and it is widely presented as gentle. Two things complicate that reputation. - The first is 甘草, licorice, which is a substantial component and carries the best-documented safety profile of any Chinese herbal article. Its active principle glycyrrhizin produces a mineralocorticoid-like syndrome: sodium and water retention, potassium loss, raised blood pressure, and in severe cases hypokalaemic paralysis and rhabdomyolysis. It potentiates digoxin, and it interacts with thiazide and loop diuretics, corticosteroids, and drugs that prolong the QT interval. The syndrome depends on how much is taken and for how long, and it arises in the setting of chronic, casual, apparently gentle use rather than of short supervised courses. People with hypertension, kidney disease or heart failure, and anyone taking digoxin or a diuretic, are the group for whom this matters. - The second is what the formula is used for. The classical 脏躁 description - tearfulness, an urge to weep or cry out, frequent yawning, behaviour 'as if possessed' - is today widely mapped onto perinatal depression, anxiety and menopausal symptoms. That mapping is an interpretation, not an equivalence, and it carries a real risk: a person in a perinatal or major depressive episode is not being adequately cared for with a three-ingredient decoction, and suicidal ideation, or thoughts of harming an infant, is an emergency that requires urgent professional care rather than a herbal prescription. Persistent low mood, loss of interest, sleep disturbance, or anxiety that is affecting a person's function belongs with a clinician. Whether this formula improves any of those outcomes has not been established by well-conducted controlled trials; the studies that exist are small and at high risk of bias, and the honest verdict is unproven. - 小麦 is wheat grain, so the formula contains gluten: it is unsuitable in coeliac disease and in wheat allergy. - 大枣 is jujube, sugar-dense, which is a consideration in diabetes. - Licorice is also avoided in pregnancy in some sources on account of the blood pressure effect. - Amounts are set by a licensed practitioner and are deliberately not given here.
