Safety. Two ingredients, and four characters of indication: The two characters 缓急 have been read in two ways, and the argument has not been settled. One reading takes them as a pair of opposites — at times easing, at times abrupt — and therefore as a description of a complaint that comes and goes. The other reading takes 缓 as a verb: to ease what is urgent, and therefore as a statement about what the formula is for. The difference is not academic. One reading describes a course; the other makes a claim about effect. This library records both readings and does not choose between them. What should not be in doubt is the category of the complaint. 胸痹 in this chapter is chest pain, and chest pain is the complaint with the least room for waiting. The first minutes go to a differential that includes acute coronary syndrome — including the presentations in which pain is not the main feature, which are common in women, older people and people with diabetes; aortic dissection, classically tearing, measured in hours, sometimes with a blood pressure difference between the arms; pulmonary embolism; pericarditis and tamponade; pneumothorax; oesophageal rupture; oesophageal spasm; costochondritis; chest wall pain; the pain that precedes a shingles rash; and pneumonia with pleurisy. A classical phrase about easing urgency is not a way through that list. The second thing worth stating plainly concerns the form in which the formula is taken. Aconite contains diester diterpene alkaloids, and the traditional safety practice around aconite is prolonged boiling, which hydrolyses those alkaloids into the far less toxic monoester and amine forms. A powder is swallowed. There is no prolonged boiling in a powder, so that step is simply absent, and everything then rests on how well the root was processed and on how much of it is used. Aconitine poisoning has no specific antidote. It produces numbness and paraesthesia, vomiting, ventricular arrhythmia and asystole, and it appears in the toxicology literature as a cause of death following the use of unprocessed or poorly processed root and of home-prepared medicated liquor. This library offers no guidance on either processing or quantity, and that is deliberate rather than an omission. 薏苡仁 is the seed of Coix, a grass, recorded as a food grain as well as a medicinal, and traditionally listed as something to be used with caution in pregnancy. This library treats pregnancy as an exclusion for this formula — not because the risk has been quantified, but because it has not been. One naming note: this formula is not 薏苡附子败酱散 (Yiyi Fuzi Baijiang San), which is recorded elsewhere for a different presentation and contains a third ingredient. Two formulas that share two of their ingredients are still two formulas, and the names are close enough that the confusion is regularly made. Chinese-language clinical literature contains observations of formulas in this group for angina, generally small and with methodological limits, and sometimes in people already receiving conventional care. The studies exist. This library does not record that benefit has been established.
