Safety. Jingui Yaolüe records this three-ingredient pill in its chapter on women's disorders during pregnancy, and its indication takes eleven characters: 妊娠小便难,饮食如故 — difficult urination in someone who is pregnant, whose eating and drinking are unchanged. That trailing clause is doing real work. The classical author is saying this is not a person who is systemically unwell; it is a pregnant woman with one local complaint. The observation is astute, and it is also exactly the point at which the classical frame and modern practice diverge. Difficult or frequent urination in pregnancy is not self-triage today. Urinary tract infection in pregnancy is treated rather than watched, because untreated bacteriuria is followed by pyelonephritis and preterm labour — which is why antenatal care screens for asymptomatic bacteriuria. Next to infection sit ureteric stone, which is harder to diagnose in pregnancy and is looked for with ultrasound rather than CT; physiological compression and hydroureter from the gravid uterus; gestational diabetes presenting with increased urination; and urinary retention. Which of these a person has changes what happens next, and symptoms alone do not reliably separate them. So the honest mapping for 小便难 here is not "damp-heat in the lower burner"; it is "exclude infection and obstruction first". Then comes the disagreement this formula puts on the table. Modern Chinese materia medica lists Sophora flavescens among the herbs used with caution or avoided in pregnancy, and there are reports of liver injury associated with its alkaloids and with matrine preparations. The classical text prescribes it to a pregnant woman. This library does not choose a side. It records both, because smoothing the contradiction away would be the dishonest move. Two adjacent naming traps also deserve their own sentence: 苦参 is Sophora flavescens, while 山豆根 — Sophora tonkinensis, which sometimes appears under similar names — is a different root with a far more serious poisoning literature, including reports of lasting neurological damage. Confusion between them is documented. The second ingredient carries an identity problem of its own. 贝母 divides into 川贝母 and 浙贝母, different fritillaria species with distinct classical uses and different alkaloid profiles, and both are confused with 土贝母, which belongs to a different plant genus altogether. All contain verticine-type steroidal alkaloids. One further classical rule belongs here, because of where it lands in this same batch: the traditional incompatibility system lists 贝母 as contrary to 乌头, the herb leading the formula immediately above this one. The third, Angelica sinensis root, has case reports of interaction with warfarin and reports of photosensitivity from furanocoumarin constituents. Three ingredients, no glamour — and this entry is the clearest instance in the library of an unresolved, documented disagreement between a classical indication and modern pregnancy guidance.
