Safety. The line that records this formula is nine characters long, and the two clauses that carry the most weight for a reader today are 不得卧, unable to lie down, and 心痛彻背, pain in the heart region that passes through to the back. 不得卧 is the clause that changes what kind of complaint this is. A person who cannot lie flat because the breath fails when horizontal is not describing a pain that is better or worse at rest; they are describing orthopnea, and orthopnea together with waking at night short of breath is recognised today as a feature of left heart failure and pulmonary congestion. It sits beside the chest-pain pathway rather than inside it, and it is measurable — oxygen saturation, a chest radiograph, natriuretic peptides, an echocardiogram. 心痛彻背 is a description of radiation. Radiation to the back appears on the modern list in more than one place, and one of them is the entry whose clock runs in hours rather than days: aortic dissection, classically tearing in quality, sometimes with a measurable difference in blood pressure between the two arms. Chest pain is the presenting complaint with the least room for delay. The differential worked through in the first minutes includes acute coronary syndrome — including the presentations that do not match the textbook picture, since women, older people and people with diabetes may report breathlessness, nausea, fatigue, or discomfort in the jaw or upper abdomen rather than crushing central pressure; aortic dissection; pulmonary embolism; pericarditis and pericardial tamponade; pneumothorax; oesophageal rupture; oesophageal spasm and reflux; costochondritis; the pain that precedes a shingles rash by several days; chest wall muscle and bone; panic; and pneumonia with pleurisy. The purpose of listing them is not to alarm but to be plain: none of them is sorted out by reading a classical text, and several of them are time-critical. The materials carry their own record. 瓜蒌 is the fruit of Trichosanthes; the root of the same plant is 天花粉, and trichosanthin, the protein isolated from that root, was investigated for terminating pregnancy and for ectopic pregnancy. Two parts of one plant, used as two different materials — and pregnancy is an absolute exclusion here. 薤白 is Allium, the genus of onion and garlic; possible antiplatelet effects are discussed in the literature, and this library records that the discussion exists without settling it. 半夏 is used only after processing; the raw tuber is toxic, and the classical tradition treated that processing as indispensable rather than optional. The fourth material is alcohol, which is itself a drug, with its own interactions, and with whole groups of people for whom it is not acceptable in any quantity. Chinese-language clinical literature contains observations of this formula in angina and coronary heart disease, usually in small studies, frequently as something added to conventional management rather than used instead of it, and with methodological limits that the reports themselves describe.
