Safety. Shen Su Yin is recorded in Taiping Huimin Hejiju Fang. It is the second formula in this batch written for a person who is not strong, and it is the one most likely to be taken without advice, because the situation it describes is so ordinary: an elderly or convalescent person, or someone with a long illness, catches a chill and has cough with phlegm. The formula pairs release with support. Ren shen carries the support; zi su ye and ge gen release the exterior; ban xia, chen pi, fu ling, qian hu and jie geng address phlegm and qi; mu xiang moves; gan cao, sheng jiang and da zao harmonise and steady the middle.
The classical reasoning is the same one that governs Bai Du San, and stating it twice is deliberate: in a deficient person, dispersing alone is held to drain what support remains. What should give a modern reader pause is how precisely that describes the group for whom a chill is least trivial. Older people, people recovering from illness, people with chronic lung, heart, kidney or diabetic disease, pregnant women and people on immunosuppressive treatment are all in the named group, and they are also the group in whom an apparently mild respiratory infection most often is not mild. Several of them have time-limited treatment windows: antiviral therapy for influenza and, for those at highest risk, for COVID-19, has to be started early to do anything. Fever with breathlessness, chest pain, confusion, or a non-blanching rash belongs on an emergency pathway, and so does breathlessness at rest in someone who was walking last week.
Three naming and safety points must be stated. First, mu xiang in this formula is the Asteraceae root: it is not qing mu xiang, the Aristolochia root that contains aristolochic acid and that is no longer carried as an official medicinal. Confusing the two is exactly the kind of error that has caused harm, which is why this library names both. Second, raw ban xia is acrid and strongly irritant to mucous membranes; dispensing practice uses only processed ban xia, and ban xia also appears in the classical list of herbs said to oppose aconite. Third, gan cao carries the risk of apparent mineralocorticoid excess with daily intake. The evidence on ren shen and warfarin is mixed and is reported as mixed. This library does not claim the formula releases the exterior, resolves phlegm, stops cough, or treats colds, influenza, bronchitis or any specific disease.
