Safety. Chuan Xiong Cha Tiao San is recorded in Taiping Huimin Hejiju Fang, the Song-dynasty official formulary, and its name carries a piece of instruction that most formula names do not: it is to be taken with tea. The tea is not merely a vehicle. In the classical reading, tea is cooling and descends, and it is there partly to check the drying warmth of what is otherwise a formula made almost entirely of wind-dispersing herbs. A formula that says how to take it inside its own name is worth noticing, because it means the vehicle was understood as part of the action.
The composition is the clearest surviving sample of what is called dividing the channels in herb selection. Ligusticum, schizonepeta, angelica dahurica, notopterygium, saposhnikovia, asarum and mint are all herbs said to release the exterior and disperse wind, but each is assigned a region: notopterygium to the greater yang channel at the back of the head, angelica dahurica to the yang brightness at the forehead, ligusticum to the lesser yang and terminal yin at the sides and crown, asarum to the lesser yin. Read as a map, the formula is an argument that headache can be located and that the herb follows the location. Read as pharmacology, seven aromatic herbs with overlapping volatile-oil content is a different object entirely. Both readings are legitimate; this library does not claim the map has been confirmed.
What the formula was used for — headache with chills, fever, nasal congestion or a stiff neck, in the classical framing an exterior wind pattern — is where a modern reader needs to stop and separate things. Headache is one of the symptoms with the longest and most time-critical differential in medicine. A headache that reaches full intensity within seconds, the thunderclap headache, is subarachnoid haemorrhage until proven otherwise. A new headache after the age of fifty, a headache with fever and neck stiffness, a headache with papilloedema or focal neurological signs or one that is worst in the morning and worsened by coughing, and a headache in someone who is immunosuppressed or has a cancer history each point to imaging or lumbar puncture rather than to herbs. Temporal arteritis in an older adult, presenting with jaw claudication or visual change, can cause permanent blindness and is measured in days. Headache with high blood pressure in pregnancy is a pre-eclampsia question. This library does not present the formula as a treatment for migraine, tension headache, sinusitis or any named cause of head pain.
One further modern point belongs beside any headache formula: medication-overuse headache. Using acute painkillers on ten or more days a month for several months can itself sustain a daily headache, and the way out is supervised withdrawal rather than a stronger remedy. That trap applies to any remedy used repeatedly for head pain, not only to this one.
Safety notes. Asarum is the most serious item on this list. Xi xin is a member of the Aristolochiaceae, the birthwort family, and contains aristolochic acid — the compound behind aristolochic acid nephropathy and the associated urothelial cancers. There is no established safe threshold for cumulative aristolochic acid exposure, and the Chinese Pharmacopoeia restricts asarum to the root and rhizome precisely because the aerial parts carry more of it. Traditionally the herb is also decocted for a long time. Mint and its volatile oil contain menthol; menthol-containing preparations applied to the face or chest of infants and very small children have been associated with laryngospasm, which is a reason for caution with concentrated mint products in that age group quite apart from this formula. Ligusticum here is Ligusticum chuanxiong, which is not interchangeable with the North American plants sharing the genus name. Honey-fried licorice carries the glycyrrhizin risk this library repeats: low potassium, raised blood pressure and oedema, worse with longer use and with diuretics, digoxin or corticosteroids. Notopterygium is a wild-harvested root with genuine supply pressure. Schizonepeta and saposhnikovia are comparatively mild. The tea used as the vehicle contains caffeine and tannins, which is a small but real consideration for anyone counting caffeine. This library gives no amounts and no instructions for use.
