Carpesium Fruit hè shī · 鹤虱

Carpesii Fructus (Carpesium abrotanoides L. — the dried ripe fruit)

Nature and flavour. Bitter, acrid, neutral; counted as slightly toxic. Spleen and Stomach systems.

The drug is the small ripe fruit of Carpesium abrotanoides, a plant whose aerial part is used separately as 天名精. Traditional texts describe it as killing parasites and reducing accumulation, and it is recorded for roundworm, pinworm and tapeworm, and for the abdominal pain attributed in traditional terms to worm accumulation. It is one of the older items in this category and appears in the Tang and Song materia medica. The recorded indications are traditional and are not established by controlled human trials.

性味归经。苦、辛，平；有小毒。归脾、胃经。

用药部位是天名精的干燥成熟小瘦果 —— 同株的地上部分另作「天名精」入药。传统记载为杀虫消积，用于蛔虫、蛲虫、绦虫，以及传统所谓虫积腹痛。它是本类药中较早的一味，唐、宋本草已有记载。所载用途属于传统记载，未被对照人体试验所证实。

Safety. Counted as slightly toxic in Chinese materia medica; nausea and abdominal discomfort have been reported. Avoid in pregnancy and breastfeeding, and avoid in young children except under the direction of a qualified practitioner. Avoid where the Spleen and Stomach are weak and cold. Not for self-use. The abdomen is not a place for trial and error: persistent or severe abdominal pain, vomiting, blood in the stool or an unexplained loss of weight all require medical assessment, and suspected worm infection should be confirmed by stool examination rather than assumed.

中药文献列为有小毒；曾有恶心与腹部不适的报告。孕期与哺乳期禁用；幼儿除在有资质执业者指导下外避免使用；脾胃虚寒者不宜。不作自用。腹部不适不宜靠试药来判断：持续或剧烈腹痛、呕吐、便血、不明原因消瘦都须就医评估；可疑的寄生虫感染应以粪便检查确认，而不应假定。
