Safety. Yue Ju Wan is the formula Zhu Zhenheng's Danxi Xinfa gives for 'the six stagnations' - qi, blood, phlegm, fire, dampness and food - and the first thing worth noticing is that there are only five herbs in it. The classical answer to that arithmetic is that qi stands first: the tradition reads constraint of qi as the condition out of which the other five develop, so moving qi is treated as the entry point rather than as one item on a list. Which herb is assigned to which stagnation is not uniform between commentaries; the common mapping is cyperus for qi, Sichuan lovage for blood, atractylodes for dampness, medicated leaven for food and gardenia for fire, with phlegm addressed through the qi and dampness herbs rather than by a herb of its own. That mapping is a teaching device inside a Yuan-dynasty nosology, not a claim about six separate modern diseases. 'Constraint' here covers presentations that today would be sorted into mood and anxiety disorders, functional gastrointestinal disorders, chronic fatigue states, premenstrual symptoms and any number of others, and the classical framework has no method for telling them apart. This matters most where mood is concerned. Stagnation is not depression, and nothing here treats depression: persistent low mood, loss of interest, and any thought of self-harm belong in professional mental-health care, and suicidal thinking is an emergency. The individual herbs carry their own cautions: - Gardenia fruit's iridoid constituents, geniposide among them, have produced liver toxicity in animal and laboratory studies, and case reports of liver injury have been described around gardenia-containing preparations, so people with liver disease or on hepatically cleared medicines should not assume it is inert. - Medicated leaven is fermented with wheat and therefore carries gluten, which rules it out in coeliac disease. - Sichuan lovage is used cautiously alongside anticoagulants and antiplatelet drugs and is traditionally avoided in pregnancy. - Cyperus and atractylodes are aromatics whose long-term use is not well characterised. One last practical point about names: this formula is also called xiong zhu wan, and modern products carrying the name Yue Ju Wan do not all contain the same five herbs. Read the label rather than the name.
