Safety. Da Ding Feng Zhu comes from Wenbing Tiaobian, Wu Tang's nineteenth-century treatise on warm disease, and it sits at the end of a chain rather than standing alone. The base is jia jian fu mai tang — the honey-fried licorice decoction with the warming and moving ingredients removed and white peony added. To that are added the three shells: turtle shell, turtle carapace and oyster shell. Then donkey-hide gelatin and egg yolk are added last. The three shells are heavy, mineralised animal materials traditionally described as anchoring and settling what has risen; the gelatin and the yolk belong to what classical language calls flesh-and-blood products, used because the depletion being addressed is understood as a loss of substance rather than a blockage to be moved. The pattern being treated, wind stirring inwardly from depleted yin, is a piece of classical mechanism and not a diagnosis. In practice what it described was tremor, twitching or carpopedal spasm late in a prolonged febrile illness, with exhaustion and a weak pulse. The honest modern translation is that this is a differential, and a broad one. Twitching and tremor arise from electrolyte disturbance — low calcium, low magnesium, low sodium; from hepatic and renal encephalopathy; from focal seizures; from drugs, including the extrapyramidal effects of antipsychotics, metoclopramide and some antihistamines; from withdrawal states; from hypoglycaemia; and from thyrotoxicosis. In a child a febrile seizure is common and usually benign, but fever with a seizure still has to be distinguished from meningitis and encephalitis, and neck stiffness, altered consciousness or a rash that does not blanch is measured in minutes. A first seizure in an adult is a reason for imaging and electroencephalography, not for a framework about wind. This library does not present the formula as a treatment for epilepsy, Parkinson's disease or any movement disorder. The safety list is dominated by the fact that this is a formula made largely of animal materials. Turtle shell and turtle carapace come from turtles, and most turtle species are listed under CITES, which makes international trade in them a permit matter as well as a conservation one — and means an unlabelled product carries an origin question with it. Donkey-hide gelatin is made by boiling donkey hide; it is an animal product unsuitable for vegetarian, halal or kosher use. Egg yolk is classically added near the end rather than boiled through, and raw or lightly cooked egg carries a salmonella risk that matters most for older adults, pregnant women and immunosuppressed people. Oyster shell is a marine shell, and near-shore bivalves concentrate heavy metals. Hemp seed is the seed of the hemp plant and is a food; it is not a way of consuming cannabis, but it can produce a positive urine test for cannabinoid metabolites in some people. Among the herbs, schisandra has been shown in laboratory work to inhibit CYP3A4 and P-glycoprotein, and interactions with tacrolimus, ciclosporin and some chemotherapy agents have been studied — that is a reason to tell the prescribing clinician rather than a reason for alarm. White peony and its standardised glucoside fraction have been associated with liver enzyme elevation and diarrhoea. Honey-fried licorice carries the familiar glycyrrhizin risk of low potassium, raised blood pressure and oedema.
