Blood-invigorating drug (breaking blood and moving qi); traditionally also classed among the qi-moving, accumulation-dispersing drugs

Bur-Reed Rhizome sān léng · 三棱

Rhizoma Sparganii — Sparganium stoloniferum (Buch.-Ham. ex Graebn.) Buch.-Ham. ex Juz., the dried rhizome

Nature and flavour. Pungent and bitter, neutral. Liver and Spleen systems.

The rhizome of a marsh plant, and one half of the most famous pairing in this category: 三棱 and 莪术 are quoted together so consistently that the classical literature treats them almost as one phrase, 三棱 breaks the blood and 莪术 moves the qi, and the argument is that stasis is rarely only one of the two. Classical sources describe it as breaking blood, moving qi, dispersing accumulation and settling pain, and record it for amenorrhoea and painful periods held to be blood stasis, for postpartum stasis, for hard abdominal masses, and for the distension and pain of food accumulation with qi stagnation. Its traditional role in food accumulation is why some materia medica classify it as a digestive drug as much as a blood drug. Modern clinical evidence for these uses is limited and is not established.

Safety. A breaking drug, not a tonic, and the tradition is blunt about it: it is avoided in pregnancy, and the classical texts also say it is not to be used where there is no stasis — the reasoning is that a drug which breaks stasis will consume what is sound when there is nothing to break. Not for anyone with a bleeding tendency, with an active ulcer, with very low platelets, or before surgery; the interaction concern with warfarin and other anticoagulants is documented, and it is not a drug to take alongside antiplatelet medicines on one's own judgement. It is also not a drug to take long-term on one's own initiative: the classical category of breaking drugs assumes a course, a supervisor and an endpoint, and none of those three can be supplied by a reader. Not a self-care herb; any use belongs under the supervision of a qualified practitioner.
