Safety. Xie Qing Wan is recorded in Xiaoer Yao Zheng Zhi Jue. Seven ingredients: angelica root, gentian root, ligusticum rhizome, gardenia fruit, rhubarb, notopterygium rhizome and saposhnikovia root. Green is the colour the tradition assigns to wood and to the liver, so the name says plainly which organ the formula is aimed at; the companion formula that drains the yellow drains the spleen. Two disambiguations come before anything else. First, this is not Long Dan Xie Gan Tang. The two share gentian and gardenia, but they are different formulas with different compositions, and the confusion is worth heading off because Long Dan Xie Gan Tang carries its own modern safety history: batches in which the mu tong ingredient was supplied as Aristolochia manshuriensis caused aristolochic acid nephropathy and later urothelial carcinoma, the cluster reported in Belgium in the 1990s, after which that aristolochia-containing form was prohibited and replaced. Xie Qing Wan contains no mu tong of any kind. The two names are close enough that a reader who has heard the aristolochic acid story deserves to be told which formula it attaches to, and which it does not. Second, it contains rhubarb. Rhubarb is an anthraquinone stimulant laxative. With prolonged use that means dependence, potassium loss that compounds with diuretics and with digoxin, cramping, and the brown pigmentation of the bowel wall seen at endoscopy. It is the reason this is not a formula to take indefinitely, and it is one more reason it is not a formula for undiagnosed abdominal pain. What the source aims it at is depressive liver fire: red and painful eyes, irritability, and in children fright-wind and convulsions. Both of those warrant a modern sentence, and the second warrants a loud one. Convulsions in a child are an emergency pathway. Nothing in this entry is a way of managing one. The differential includes febrile seizure, epilepsy, intracranial infection such as meningitis and encephalitis, electrolyte disturbance including low sodium, calcium and magnesium, hypoglycaemia, head injury, breath-holding spells and syncope, and toxic ingestion. The classical text was written before electroencephalography, lumbar puncture and bedside glucose measurement existed. A child who has had a seizure needs to be assessed, and a first febrile seizure is a reason to be seen even when the child looks entirely well afterwards. Never give anything by mouth, including any herbal liquid, to a child who is not fully awake, because a swallowing reflex that is not intact turns a drink into an aspiration. A red, painful eye is not liver fire until the alternatives are excluded. Conjunctivitis is common and usually benign. But uveitis brings pain, photophobia and reduced acuity and needs same-day ophthalmology; acute angle-closure glaucoma presents with severe pain, blurred vision with haloes around lights, vomiting and a fixed mid-dilated pupil, and damages the optic nerve within hours; scleritis and orbital cellulitis are urgent; and proptosis with double vision needs imaging. Any red eye with reduced vision, severe pain or a pupil that looks wrong should be treated as urgent rather than as something to cool. The herbs: angelica root contains furanocoumarins, which photosensitise, and laboratory work reports antiplatelet activity, so caution is reasonable with warfarin and antiplatelet drugs and before surgery; it is also widely sold for women's health on an evidence base that is genuinely mixed, and this library reports it as mixed. Ligusticum rhizome has similar laboratory signals around platelets. Gentian is intensely bitter, and several Gentiana species are under wild-collection pressure. Notopterygium is likewise wild-collected and has been under pressure in its Chinese range. Gardenia carries the same limited-evidence liver note described under Xie Huang San. The pill form is bound with honey, which is a carbohydrate load and, again, never for an infant under twelve months. This library does not claim that this formula stops seizures, clears the eye or treats any disease.
